SQL Errors Should Be Resolved

WarLord spent some time on the VM, allocating /SWAP and other resources, to prevent SQL from crashing. The general consensus is that phpBB is inefficient at SQL requests, which was eventually leading to mysql running out of resources, and crashing.

With the combination of the daily reboot, the forum should now be stable.

Update on this issue:

It turned out that we were running a mysql version that had an exploit allowing external unauthorised access to crash the database, so that instability was due to DoS attacks.

If you are for any reason running mysql < 5.7.31, please update to 5.7.31 (or 8, if your software supports it) on your servers.

The update process itself was hell due to bugs in Ubuntu 18.04, but I managed to manually force it by editing the postinstall script. If you run into issues updating mysl, you can fix it by commenting out the line to invoke mysql_upgrade. The downside of this is that your system database tables will not report the correct version to PHP via mysqli, but the actual binary update works.
